How to become a supplier:
This is the process to obtain a new Supplier (vendor) number to receive Awards and Honorarium payments.
Instructions:
- Navigate to the Prospective Supplier Request form here: https://wd5.myworkdaysite.com/en-US/supplier/gatech/SUPPLIER_REGISTRATION_PROD.
- Select Sign In if you already have an account or select Create Account if you do not have an account.
- Select Home to navigate to the main page of the website. Select Register to access the supplier request form.
- Enter Your Name as the Business Name, select Individual for Supplier Category, Awards, Prizes, Fellowships for Supplier Group, and your Social Security number for Tax ID in Business Details. Skip the additional questions for Individual/Sole-Proprietor in the Supplier Category field. (If you plan to fill in additional details, then make sure that the response for the questions below is as suggested:
- What expenses will be billed to Georgia Tech in the performance of services to Georgia Tech? – DO NOT CHECK ANY BOX for this option.)
- Enter the Accepted Payment Types and Default Payment Type as Check in Payment Types.
- Check ‘Accept All Currencies’ and enter the Default Currency as USD in Currencies.
- Enter your Phone Number and Email Address in Business Contact Details.
- Enter the Country, Address, City, State and Postal Code in the Remit-To Address. (Enter the Mailing Address only if it is different from the Remit-To Address.) Click Next.
- Skip the next two pages Spend Category Hierarchies and Classifications by clicking Next.
- Enter your First Name and Last Name in the Legal Name section. Complete the financial interest question, “Do any of the following have a financial interest or ownership in your company?” as appropriate and then click Next.
- Attach tax forms:
- If you are non-US person, then attach W8-BEN form on the Attachments page: https://www.irs.gov/pub/irs-pdf/fw8ben.pdf. (additional guidance for tax forms here: https://www.irs.gov/forms-instructions-and-publications?find=w-8)
- If you are US person, then attach W9 form on the Attachments page: https://www.irs.gov/pub/irs-pdf/fw9.pdf
- Check Box and enter Your Name as Signature in the Signature page. Click Next.
- Review the Summary page to make sure your contact information is correct. (Use the back button for corrections.) Your award check will be mailed to the address you entered, so please make sure to use an address you will be able to access after graduation.
